The person whose name appears on the credit card should complete and sign the form, then fax it to the telephone number that appears on the form. Please do not complete and fax this form unless you have been asked to do so by one of our travel consultants. We will confirm the amount to be charged to your credit card either by email, fax or phone.

FOR IDENTIFICATION PURPOSES, THE CREDIT CARD AUTHORIZATION MUST BE ACCOMPANIED BY A COPY OF YOUR VALID DRIVERS LICENSE OR PICTURE ID, PLUS A COPY (FRONT & BACK) OF THE CREDIT CARD BEING USED TO PAY FOR YOUR TRIP. WE WON'T CHARGE YOUR CREDIT CARD WITHOUT THIS INFORMATION.

JUST ONE MORE THING: If you are a US citizen traveling internationally, you must have a valid passport in order to board the plane or cruise ship. Let us be very frank, everyone, who plans to travel internationally, should have a passport. Many countries now require a passport for entry.

Please visit the US State Department website for accurate and up-to-date visa requirements and information on your destination. For domestic travel, you will need to present your photo ID, for example, a driver's license, in order to board the plane.

Non-US citizens must have a valid passport, their alien registration card (in order to re-enter the United States) and necessary visas. Contact the embassy or consulate of your destination country for correct visa information.

Travelers from other countries should contact the embassy or consulate of their destination country for visa requirements.
